Verbose installation procedure?

The issue you describe should really not happen. It points to a disk partitioning/writing error. I suspect this could be related to the trying to get the hp G5 cciss block driver working on your system as it is not supported by new kernels. I would test the original iso on another hardware and see if you still get the issue, if not i would suspect the driver issue. Another possibility is the way you re-build the iso file.
Another reason i suspect this / could be related is your feedback on not being able to add new OSDs. This could also indicate an issue with partitioning failure. For v 2.0 we do have a verbose log for ceph disk logs in :
/opt/petasan/log/ceph-disk.log
it should contain a lot of detail on OSD creation process.
